Job Description: Physical Therapist
Plans and implements specific treatment programs for patients according to the principles and practices of physical therapy and in collaboration with the health care team. Reviews patient's medical history and/or chart. Ensures orders have been dated and signed. Evaluates the skilled physical therapy needs of the referred patient and develops a written Plan of Treatment in collaboration with the patient and/or other members of the health care team. Establishes short and long terms goals with specific time frames. Addresses completion of goals and establishes new goals. Updates treatment plan. Re-evaluates patient and communicates with referrer and other clinicians involved in the patient's care. Directs and aids patient in therapeutic exercises, muscle re-education, and functional training using pulleys, weights, steps, and inclined surfaces. Uses such treatments as whirlpools, hot and cold packs, electrical stimulation, manual therapy, traction, massage, wound care, phonophoresis, paraffin, gait training, ultrasound, iontophoresis, TENS, Biodex and EMG Biofeedback.
Education: Graduate of an accredited Physical Therapy Program with a Bachelor's Degree or Entry Level Master's Degree.
Experience: Experience in treating neurologically involved patients required, outpatient neuro experience a plus. Professional memberships and development of career goals encouraged.
Licensure/Certification: Licensed as a Physical Therapist by individual state. CPR required.
